Sarah Morgan's Outfit can only be unlocked on the Companion's Belongings mission if she died during the events of High Price to Pay mission quest.
This mission will be unlocked after High Price to Pay. To get the outfit, go to the Lodge's basement and there will be a Storage Crate containing the outfit!
Companion's Belongings Rewards and How to Unlock
During the High Price to Pay mission, you will be given a major choice to either go to the Eye or defend the Lodge.
If you choose to defend the Lodge, the companion you are closest to who is at The Eye will die. Alternatively, if you choose to check up on The Eye, a companion who is left defending the Lodge will die. This means that Sarah's death is dependent on how close you are to her.

If she's the one who dies in the main mission you get her outfit by going to the Lodge basement and collecting it out of the storage container by all the crafting stations. Figured I'd let you know since your guide isn't updated on where to get her costume.
