The Head Mushroom is located in a waterlogged cave in Vissudha Field, Sumeru. You need to drain the water in that cave before you can find the Head Mushroom.

How to Drain the Cave Water for the Head Mushroom

Genshin - How to Look for the Head Mushroom

Interact with the Cube Mechanism by the entrance of the cave to drain the water. The Cube Mechanism is located near two Ruin enemies and a Seelie. Defeating the two Ruin enemies will unlock the Cube Mechanism.
