You have to defeat La Signora in Chapter 2: Act 3 and finish the Archon Quest to unlock the Narukami Island: Tenshukaku Domain and the weekly La Signora challenge.

La Signora is the only enemy in Narukami Island: Tenshukaku, but the boss fight has two phases. You will fight a Cryo La Signora then move on to a Pyro La Signora.

Bring Your Best Healers!

The character in the GIF is Kokomi, who is one of the best healers in Genshin Impact.

Challenging the Narukami Island: Tenshukaku domain will feel like you're constantly freezing from Sheer Cold or burning with Pyro. Bring your best healers, like Kokomi, Barbara, or Bennett, to keep you alive!

The three healers mentioned are especially great for the Domain because their elements counter La Signora's phases.

How to Avoid Sheer Cold or Burning in Narukami Island: Tenshukaku

The image above shows two features that will help you reduce incoming damage during the boss fight. The Heart of Flame (on the right) gives off heat during Signora's Cryo Phase, while the Eyes of Frost (on the left) douses the fires of her Pyro phase.

Destroy these two right as the status effect meter on the bottom of your screen fills up so you don't take periodic damage!
