You can obtain That Which Was Once Ehecatl after speaking with Ehecatl and lighting up the interior of the Ancient Sacred Mountain during The Frozen, Rekindling Land quest. The quest item will not automatically go into your inventory and you need to pick it up in front of the mechanism.

Unlocks a Hidden Room and a Precious Chest

Genshin Impact - That Which Was Once Ehecatl altar location

You can use That Which Was Once Ehecatl to unlock a hidden room containing a Precious Chest.

To unlock the hidden room, go to the Fallingstar Fields in Atocpan, Natlan. Enter the Tephric Ruins, and continue walking until you reach the altar. Place That Which Was Once Ehecatl on the altar, and a short cutscene will play showing the location of the hidden room.

You can unlock the entrance to the Tephric Ruins during the Investigator of Ancient Ruins Quest quest.
