To unlock the Chronic Illness quest, you will need to reach Adventure Rank 35 or above and complete the second part of Chapter 3: Act 1: Further Observation!
Further Observation Quest Guide
Head to the marked location near the base of Gandharva Ville to check on Collei.
Follow Tighnari behind the house where Collei was and speak with him.
To assist Tighnari in collecting herbs, go to the marked location south of Sumeru City by the Chinvat Ravine.
Collect four Fresh Nilopata Lotuses. You can find them in the area, floating in the water!
Meet up with Tighnari again at the marked location, in the corner of the area where you first talked.
After collecting the lotus and speaking with Tighnari, Amir will appear and inform you that they have discovered a Withering Zone.
Follow Tighnari to the marked location to find the Withering Zone.
Clear the Withering Zone using a combination of the Dendrograna and Charged Attacks. You will have to defeat an enemy right after to destroy the Withering Zone.

| 1 |  Use the Auspicious Branch to summon three Dendrograna. |
| 2 |  Destroy the three Fetid Boughs. |
| 3 |  Use charged attack to destroy them. |
| 4 |  A Grounded Hydroshroom will appear after you have destroyed all of the Fetid Boughs. Get rid of the monster and the remaining fungi. |
| 5 |  Destroy the Tumor after defeating all the enemies. |
Return to Gandharva Ville after clearing the Withering Zone.
Head over to Haypatia's cave near the Chasm entrance to Sumeru.
