This build seems to be very outdated, there is tons of rotation/build guides on this, most originating from chinese servers.
I mean hold R not just R
insightful strike is better than adaptive steel, and the best rotation is q with sword to get 5 stacks, then r with spear, then q with spear (only need 8 stacks then cancel with e), then spam ~ with sword, if out of stamina to use ~ then use dragon's breath (should drink alcohol beforehand)
add a light attack between might be high ping
You need River Flow (10 stacks) from Sober Sorrow for it to re-apply bleeds.

idk man, i tried using Sweep All to refresh Stacks of bleed. and it didnt refresh them at all. Even in the video example above it doesnt refresh. Unless the UI is broken or im missing something?
You refresh the stacks with Sweep All from Spear

Hi, no more Spear Charge attack ? Sword Horizon is really better than Fivefold Bleed ?
How come no Spear Charge attack for 50% more DoT damage and Bleed damage? Goes well with Dragon's Breath + Drunken poet to Ignition (dot), no?
I must be doing something wrong because for some reason the bleed stacks are gone before I can even detonate them with the swords skill

It can drop randomly but you cant start choosing it from sword trial boxes until 56.
I dont think it starts dropping till lvl 56. It shows up as reward from sword trial then.
