Olroth's Resolve Ultimate Life Flask is obtained as a drop reward from defeating Olroth, Origin of the Fall.

He can be encountered randomly in level 79 or higher Expedition Logbooks. The entrance to his arena is marked by a skull icon on the map. You’ll need to detonate an explosive near the indicated area, which will reveal the entrance to the Kalguran Tomb where you can fight Olroth.
