Cruel Difficulty Permanent Buffs

You’ll be able to obtain all the permanent buffs again during Cruelty difficulty, except that some of the permanent buffs have been changed or completely removed. See a list of permanent buffs that have been changed in Cruel difficulty.

Candlemass' Essence dropped by Candlemass, the Living Rite now grants 8% Maximum Life in Cruel difficulty instead of +20 Maximum Life in Normal difficulty.

Ignagduk, the Bog Witch now drops the Gemcrust Skull that grants +40 to Maximum Spirit in Cruel difficulty. The Boss initially dropped the Gemrot Skull in Normal difficulty, which grants +30 to Maximum Spirit.
3 The Gembloom Skull that grants +30 to Maximum Spirit is no longer dropped by the King in the Mists during Cruel Difficulty.

Permanent buffs are bonuses that will be applied to your character for the rest of its lifetime. These bonuses come from completing side quests and/or defeating optional bosses throughout the campaign.

All of These Permanent Buffs Can Be Missed

You can play through the whole game without getting any of these buffs since they come from optional quests and bosses. Be on the lookout for these buffs during your campaign to get them!
