Special Items are randomly dropped when defeating enemies. Higher level Gems are dropped as you progress through the campaign and achieve higher levels.

Tokens, however, start dropping from enemies after you first encounter and finish an event

Tokens are Used to Enter Trials

Tokens are Used to Enter Trials
You can use Tokens to access different Trials like the Trial of the Sekhemas and the Trial of Chaos. Depending on the level of the Token offered at the start of the trial, you can run through more trials and take on harder difficulties to up the ante and rewards!

These tokens are consumed once you enter the area.

Get New Skills Through the Gemcutting Menu

Path of Exile 2 - Get New Skills Through the Gemcutting Menu
You can engrave Uncut Gems with new skills by right-clicking it to open the Gemcutting menu and selecting the skill you want, these can be Skill, Support, or Spirit skills.

Once engraved, the gem can then be equipped in your skill slots. This process cannot be reversed.
