Omniphobia, Fear Manifest may randomly spawn in Delirium maps or in higher waves during a Simulacrum at a Realmgate, with a guaranteed chance to spawn during the last few waves.
Additionally, while unconfirmed, being more delirious may increase the chances of bosses spawning during Delirium maps.

Players should make sure to dodge Omniphobia's earth and leap slams. Most of what makes Omniphobia dangerous is its high-damage dealing axe attacks, especially its AoE earth and leap slams.
To properly dodge them, players must dodge right before Omniphobia's axe touches the ground. Doing so allows them to capitalize on invincibility frames and dodge the AoE ground spikes that come with the slams. After dodging, players can use Omniphobia's recovery window to damage it.
Players should also avoid getting hit by Omniphobia's Axe Slash Projectile. Being one of two projectile attacks Omniphobia has, this projectile delivers a lot of damage, almost as much as the slam attacks.
To dodge this attack, players should dodge out of Omniphobia's line of sight the moment he spins to throw the projectile and has its back toward you.
Another attack that players should avoid is Omniphobia's vomit attack, as it deals considerable damage in a relatively wide radius. To dodge it, players must watch out for red bubbles forming around Omniphobia's head after he slightly bends down. Then, they must dodge towards his side when he lifts his head.
