You automatically get this quest upon obtaining a Breachstone.

• Use the Realmgate to Travel to the Twisted Domain
• Enter the Breach
• Follow the Breach in search of Xesht
• Defeat Xesht
• Return to Ketzuli to tell him of your triumph
