The Arbiter of Ash is considered one of the most mechanically challenging Pinnacle Bosses in Path of Exile 2. To access this encounter, you must first collect Crisis Fragments by defeating the Uber Act Bosses within the Iron, Copper, and Stone Citadels. Once acquired, these fragments unlock the path to the Burning Monolith, where the fight takes place.

The King in the Mists is the Pinnacle Boss tied to the Ritual mechanic. This boss summons multiple altars and fetishes, each applying powerful effects and overwhelming the player with various attacks.
To access this fight, you must obtain An Audience With the King, a special Invitation that can appear as a Favour reward in Ritual encounters. Use it at the Realmgate to initiate the fight.

Xesht, We That Are One is a powerful Breachlord and presents a good challenge as a Pinnacle Boss. To fight Xesht, you must use a Breachstone, which is crafted by combining a full stack of Breach Splinters dropped by Breach monsters. Once acquired, use the Breachstone at the Realmgate to access the Twisted Domain, where Xesht awaits.

Olroth, Origin of Fall is the Pinnacle Boss of the Expedition mechanic. This ancient Kalguuran is a swift warrior that uses a Kalguraan rune to weaken and slow you down.
To encounter Olroth, you must locate a Kalguuran Tomb in Expeditions with an Area Level of at least 79. If your Expedition Logbook’s area level is lower, you can still reach 79+ by allocating Logbook Difficulty nodes in the Atlas Passive Tree.

Omniphobia, Fear Manifest is one of the two Simulacrum bosses in Path of Exile 2. This boss specializes in high-damage melee attacks and leaping strikes that can quickly eliminate unprepared players.
Omniphobia can be encountered randomly in Delirious maps but only guarantees Unique drops when defeated at Wave 15 in the Simulacrum.

Kosis, the Revelation is the second Simulacrum boss, known for charging up energy and detonating them to unleash massive AoE explosions. Similar to Omniphobia, Kosis can appear in high-wave Delirious maps or at the higher waves of the Simulacrum.
Like Omniphobia, Kosis only drops its exclusive Uniques when defeated in the Simulacrum as the final boss of Wave 15.

Zarokh, the Temporal is a djiin that have master the manipulation of time. You'll encounter this boss as the final boss of the Trials of Sekhemas. Use a Djinn Barya with a level of at least 75 to gain access to the 4th trial and fight Zarokh in the trial's final room.

The Trialmaster is an ancient Vaal and servant of Chaos. To challenge him, you must first obtain three different Fate items that drop from the Trial 10 boss. Once you have the three Fate items, use them to unlock the door beyond the final Trial of Chaos chest, which leads to the Trialmaster's arena.
Pinnacle Boss fights require an Invitation, a special consumable key item, to initiate the encounter. These invitations are usually earned from the related content. For instance, facing Xesht requires a Breachstone, crafted by combining a full stack of Breach Splinters dropped from monsters in Breaches.
Once you have the invitation, you’ll typically use it on the Realmgate inside the Atlas Map to start the fight. However, some bosses, like The Arbiter of Ash or the Trial Bosses, require activation in a different location.
Breachstone Effects and How to Get
Each Pinnacle Boss guarantees the drop of one exclusive Unique Item from its loot pool. Many of these Uniques are more powerful, build-defining, and some, can be rarer than the others.
Zarokh, the Temporal is an exception, as it drops Unique Relics instead. These relics are used to access “challenge runs”, which rewards Exclusive Uniques upon clearing them.
A number of Pinnacle Boss Uniques have high variance to their modifiers, meaning their value can change drastically based on their roll. Identifying a poorly rolled Unique can significantly lower its trade value compared to its Unidentified state.
Consider whether it’s better to sell these Uniques as Unidentified or take the risk of identifying them in hopes of landing a valuable roll.
As of Patch 0.2.0, dying to a Pinnacle Boss does not longer result in experience loss. However, completing the encounter also does not grant experience as a tradeoff. This protects players who are leveling and don’t want to risk losing up to 60% of their experience dying from a boss attempt.
Defeating a Pinnacle Boss for the first time grants two Atlas Passive Points for that content's Passive Tree, letting you allocate Notables that boost rewards. The travel nodes leading to these Notables also raise the boss's difficulty, and defeating them again on a higher difficulty grants more Atlas Passive Points.
The final Trial Bosses, Zarokh, the Temporal and The Trialmaster, do not reward Atlas Passives Points as there are no Atlas Passives exclusive for their trials.
Increasing the difficulty enhances the boss’s damage, defenses, abilities, and rewards while also reducing the number of respawn attempts available. You can allocate up to four Difficulty Nodes at once—any additional nodes will have no effect.
Perhaps the most significant impact of increasing difficulty is the reduction in respawn attempts during the fight. At Difficulty 4, you will only have one respawn, down from five at Difficulty 0.
While Pinnacle Bosses can drop all their exclusive Uniques at lower difficulties, increasing the difficulty boosts the drop rate of rarer Uniques, making it a viable strategy for farming high-value items.
Pinnacle Bosses are the most challenging and rewarding encounters in Path of Exile 2. Each requires a unique access method, such as Breachstones, Simulacrum, or An Audience with the King. Defeating them grants exclusive Uniques and Atlas Passive Points for their respective content.
Pinnacle Bosses always drop at least one of their exclusive Uniques. However, Zarokh, the Temporal, is an exception, as it instead drops Unique Relics that increase the difficulty of future Trials in exchange for exclusive rewards.
Each Pinnacle Boss drops a different kind of Book of Knowledge, granting two Atlas Passive Points for their respective content. For example, Olroth, Origin of Fall grants points for the Expedition section of the Atlas Passive Tree.
