Twisted Domain is a special map in Path of Exile 2 (PoE 2) that is linked to the endgame's Breach mechanic and contains a powerful boss. See a guide to the Twisted Domain and its location, drops, and boss here!

Players should kill monsters as soon as they appear to avoid getting swarmed. Getting swarmed in the Breach phase will slow players down and put them in a dangerous position. Additionally, immediately killing monsters allows players to only worry about their front and not their backs or sides.

Players should attempt to stay relatively close to the edge of the breach. Doing so allows them to kill monsters immediately, quickly figure out where the right path is, and smoothly follow it. Additionally, it makes the phase shorter as the sooner players reach the boss entrance, the sooner it finishes.

Players should interact with any Clasped Hands they encounter during the Breach phase. Doing so increases the rewards they receive upon completing the Breach phase.

Players can increase the Twisted Domain's difficulty for better and rarer rewards when finishing the Breach phase or defeating the boss. They can do so by unlocking the appropriate nodes in the Breach Atlas Tree.
However, note that doing this makes monsters more powerful and decreases the available respawn attempts, making the Twisted Domain less forgiving. As such, players should only do difficulties they know they can handle.

As most, if not all, enemies in the Twisted Domain deal chaos damage, players should max out their Chaos Resistance to ensure survivability. This tip especially applies to players who rely on energy shields, as Chaos attacks deals double damage to energy shields.

Players can enter the Twisted Domain by inserting a Breachstone in the Realmgate. Players can obtain a Breachstone by accumulating 300 Breach Splinters while running Breach maps.

Players may obtain the following Unique Items when defeating the Twisted Domain's boss. Some items only drop or have higher chances of dropping at higher Twisted Domain difficulties.

Running a Twisted Domain map can also give players multiple rare and valuable items. Listed above are some of the most notable ones.

Xesht, We That Are One is the pinnacle boss in the Twisted Domain, which players can fight if they successfully clear the map's breach phase. It specializes in dealing with chaos damage and hitting players with numerous AoE and projectile attacks that are hard to dodge.

The Twisted Domain is a Unique Breach map accessible by a Breachstone. It is unique as it expands on the normal Breach mechanic on Atlas maps and has two phases that players must finish to complete the map.

The Breach phase is the first phase players must complete. It is similar to a regular Breach wherein players must interact with a large hand to start it. This time, however, a 45-second timer will begin counting down once it starts.
Players can add more time by killing enemies. If the timer hits zero, players will fail this phase and the map unless they have a respawn attempt.

Players must reach the boss room entrance by following the linear path to complete the Breach phase. When the phase is over, multiple drops and rewards will spawn. On this note, monsters in this phase do not drop any items.

Once players finish the phase, they can fight Xesth, We That Are One, in its boss room. Defeating it will grant high-rarity rewards and unique and exclusive drops.

As of the 0.1.1c patch, players get five respawn attempts. If players fail or die during the Breach phase of the map, they get teleported to the start of the map. Meanwhile, if players die while fighting Xesht, they are teleported to the boss room entrance.
Additionally, monsters will not grant additional exp for reattempts during the Breach phase.
