Omen of Corruption is obtained as a possible drop reward from defeating the endgame variant of The King in the Mists. You will not be able to get this item if you fought the campaign version of The King in the Mists.

Omen of Corruption is purchased using Tributes after completing a Ritual encounter. Tributes are earned by killing waves of enemies within the ritual circle.

Omen of Corruption can be obtained by visiting the Currency Exchange NPC in every Act. Currency Exchange becomes available after completing Act 3.
Omen of Corruption is used to modify certain aspects of crafting by alternating the properties of specific Currency Items either by limiting removal or adding more modifiers to your specific item.
To use Omen of Corruption, right click on it while it is in the inventory to activate it. The Omen of Corruption should be active if it is highlighted by a red border, meaning its effects will take effect upon meeting the condition. Deactivate the item by right clicking on it again to prevent consuming the item.

After right-clicking the Omen of Corruption to set it to be active, use a Vaal Orb on an item to alter its modifiers, consuming the Omen of Corruption in the process.
You can either get better modifiers such as exceeding the maximum limit of unique modifiers on equipment, or worse, lowering the modifiers. The effects cannot be reversed, so proceed with caution when corrupting equipments.
