Once you find a Realmwallker, activate the portal to awaken it and defeat the hollow mobs it spawns nearby while its walking towards its destination. Keep in mind that you cannot damage the Realmwalker itself during this phase so focus on clearing the waves until it reaches it destination.
Along the way, Bloodbound Guardians will spawn in between waves while you are clearing the mobs. Defeat them to decrease the Realmwalker's health and make it move faster while it is still invulnerable to make things easier once you start dealing with the Realmwalker itself.
When the Realmwalker finally reaches it destination, it will spawn 3 Hatred Spires nearby that requires you to destroy them to remove the Realmwalker's invulnerability. This will allow you to damage the Realmwalker's health directly so try to save up your high damage skills before then.
Once all Hatred Spires are destroyed, the health bar will turn red and you can begin damaging the Realmwalker itself. This is the final phase of the Realmwalker and defeating it will drop a portal to the Seething Realm.
Once you have defeated a Realmwalker, a portal to the Seething Realm will drop on its location. This is a two-part dungeon that offers various rewards, specifically the Seething Opal elixirs.
