Sacred and Ancestral items in Diablo 4 (D4) are stronger versions of regular items found in World Tier 3 and higher. Read on to learn more about Sacred and Ancestral items, how they work, and more about them!

In Season 2 of Diablo IV, Blizzard has increased the iPower ranges for both Sacred and Ancestral gear. When dropped by a monster, Sacred gear should always be somewhere in between 625-750, while Ancestral gear can now drop with 780-925 item power.

Sacred and Ancestral items are more powerful versions of regular item drops, featuring higher stat rolls and Item Power. From our observations, these items are around one to two Breakpoint Tiers higher than their regular counterparts.

Note that Sacred and Ancestral items aren't counted as exceptions when selling and salvaging items. To avoid accidentally disposing of valuable gear, we recommend using the Mark as Junk mechanic in the inventory and only selling marked items.
At the Occultist, you can imprint Legendary Aspects onto Rare Sacred and Ancestral gear, allowing you to boost their power even further.
Aside from the Common, Magic, and Rare rarities, it's also possible to find Sacred and Ancestral versions of Legendary and Unique items. These versions feature potentially stronger effects and stat rolls, making them incredibly sought-after in the late game.
In addition to these characteristics, Sacred and Ancestral items also have a higher chance of dropping in Nightmare Dungeons compared to other content in the game, as confirmed by Associate Game Director Joseph Piepora on Twitter.
To make Sacred items start dropping in the world, you first need to reach World Tier 3 (Nightmare difficulty). You can reach this World Tier by finishing Diablo 4's main campaign and completing the Cathedral of Light Capstone Dungeon in Kyovashad.
To make Ancestral items start dropping in the world, you need to unlock World Tier 4 (Torment difficulty). You can unlock this World Tier by completing the Fallen Temple Capstone Dungeon at Chambatar Ridge in the Dry Steppes region.
