Fugitive Heads can be farmed by defeating Headrotten bosses in Diablo 4 (D4) Season 7 and are used to trade for Occult Gems or Restless Rot. See how to farm Fugitive Heads and what they are.

Fugitive Heads can only be obtained once you have reached Torment Difficulties. Each difficulty has a small chance for them to drop from Headrotten Bosses but increases up to 10% on Torment IV.
You can also use the Draught of Whispers elixir to essentially increase your chances by 50%. Keep in mind that this elixir only has a 1 hour duration and can be stacked with other elixirs.
How to Get Draught of Whispers

You can farm Fugitive Heads by completing Whisper bounties in Headhunt zones. Headhunt zones are similar to Helltides where you can do the new Whisper bounties.
Completion of Whisper bounties will reveal Exposed Roots in Headhunt Zones which will have a chance to spawn Uprooted Cocoons and Headrotten bosses.

Defeat Headrotten bosses for a chance to get Fugitive Heads. Headrotten bosses are quite strong and it's recommended to take them on with a party or other players present in Headhunt zones.
Alternatively, you can find Headrotten bosses inside Rootholds which are seasonal dungeons within Headhunts. Rootholds will always have a boss at the end but it's not guaranteed that the boss is a Headrotten.

Fugitive Heads are used to trade for materials with Gelena at the Tree of Whispers. Occult Gems are seasonal socketables that provide unique abilities and have to be crafted first with varying material requirements.
All Occult Gems provide 160 Armor and 8% Resist All on top of their unique effects. Higher tiers of Occult Gems are crafted using the following materials: Fugitive Heads, Restless Rot, Gem Dust, and other Occult Gems.
