As of writing, new clues have been found pointing towards the existence of the cow level in Diablo 4. After Season 2 dropped, killing 666 cows gave you a secret Cow Relic that you needed to assemble alongside 2 other relics to reach the Forlorn Hovel cow level.

What Is the Cow Level?

Joke Level in Previous Games

The Cow Level was a recurring joke level or event in previous Diablo games. During the time of the first Diablo game, the existence of a secret level that would appear if you clicked on a specific cow enough times was a popular rumor.

While it didn't exist at the time, Blizzard eventually added secret Cow Levels into Diablo games as a nod to this rumor. In particular, there was the Jersey's Jersey quest in Diablo: Hellfire, the Secret Cow Level in Diablo 2, and Not The Cow Level in Diablo 3.
