The Undercity: Starving Pride quest is automatically unlocked by completing Undercity: The Promise.

This quest is part of the Kurast Undercity quest line that unlocks the Kurast Undercity mechanic when completed.

Defeat Longtooth in the Cave District

Defeat the Infected Lacuni Scout
Follow Sihek around town and enter the Spiritborn Sanctum. Defeat the Infected Lacuni Scout and speak with Sihek again.

Diablo 4 Vessel of Hatred - Withered Lacuni
Head to the quest area and collect 10 Insect Husks from Afflicted Lacuni. Afterwards, search the Abandoned Lacuni Camp to the north. Return to Sihek to unlock the Cave District.

Longtooth, the Wretched
By now, you should be familiar with the drill. Head to the Spirit Brazier, enter the Cave District, and reach the district boss before time runs out.
Defeat Longtooth, the Wretched and speak to Ormus in the Council Chambers to complete this quest and unlock Spirit Brazier Tributes.
