How to Beat Blood Maiden

The Blood Maiden is a new boss you can summon during the Helltides in Diablo 4 (D4). Read on to learn more about how to defeat the Blood Maiden, including her location and all attack patterns!

How to Summon Blood Maiden

Offer Baneful Hearts at an Accursed Ritual

In order to summon Blood Maiden, look for the Head with Horns icon in the Helltides on your map and head towards that location. An Accursed Ritual can be found nearby, and you can offer a Baneful Heart on each Altar to start the ritual.

Clear the mobs until the demon egg in the center explodes and a Blood Maiden spawns. Keep in mind that she spawns at the end of the Seasonal Questline.

How to Beat Blood Maiden

Evade the Rain of Fire

Blood Maiden will throw multiple fireballs simultaneously on the ground, with one fireball that locks on to you. Once she starts throwing them, reposition yourself away from the rings of fire. While she is throwing the other fireballs, she will throw the one locked on to you in the middle of her attacks. Once she does, evade away.

Defeat Blood Maiden's Hellborne Disciples

On top of the fireballs, Blood Maiden will fly up, throw 4 fireballs from above, and land back where she started. While she is above, her health bar will disappear, and she cannot be damaged or targeted. Eliminate all 4 of the Hellborne Disciples as fast as you can to avoid facing multiple Elites since these Disciples can pack a punch in higher difficulties.

Evade Towards the Fire Wave

When you are in close range, Blood Maiden will jump towards a target location and slam the ground with a wave of fire. While it increases in size, evade towards the center and continue to deal damage to the Blood Maiden.

Blood Maiden Rewards

Cinders and Unique Items

Defeating Blood Maiden rewards you with a huge amount of Abberant Cinders and tons of Unique equipment and Temper Manuals. Keep in mind that you can do continuous runs of Blood Maiden as long as you have 3 Baneful Hearts.

Drops Exquisite Blood

The Blood Maiden can drop Exquisite Blood when you defeat her. This is a requied item used to summon the Ladder Boss, Lord Zir.

Found in Accursed Rituals During Helltide

Accursed Rituals can only spawn during Helltides. To make this icon appear on your map, you have to be near its location. Once the icon has appeared, it will be marked on your map for the entire duration of the Helltide.
