How to Get Malignant Hearts (Season 2 Updated)

Malignant Heart is one of the four Malignant Body Parts needed to summon the Echo of Varshan in Season 2 of Diablo 4 (D4). Read on to learn more about the Malignant Heart, how to get it and farm, how to use it, and so much more.

How to Get Malignant Hearts

Whispers of the Dead Caches

Similar to other Malignant Body Parts, you can obtain the Malignant Heart by completing the Whispers of the Dead quest and opening the cache reward, which contains a random Malignant Body Part (Gurgling Head, Trembling Hand, Blackened Femur, and Malignant Heart).

However, it's important to note that, out of all the Malignant Body Parts, the Malignant Heart is only available on World Tier 4, as it is required for summoning the Echo of Varshan in Torment difficulty.

How to Complete the Tree of Whispers

Kill the Grotesque Debtor

Killing a Grotesque Debtor guarantees a random Malignant Body Part. You can easily find them in areas with Grim Favor rewards and areas with the Blood Harvest Event. This means you can grind for this Elite enemy while completing your Whispers of the Dead quest.

Transfers to Your Stash

If you miss a Malignant Body Part drop, it will automatically be transferred to your stash. Be sure to check the Missed Equipment tab in your stash periodically.

How to Use Malignant Hearts

Summons Echo of Varshan

Malignant Body Parts are used to summon the Echo of Varshan inside the Malignant Burrow, located in The Writhing Mire beside the Tree of Whispers in the Hawezar region. Do note that the Malignant Heart is only required when summoning the Echo of Varshan in World Tier 4.

Since the Malignant Heart is the rarest Malignant Body Part of all of them, you can use it as a crafting material to craft any of the other three Malignant Body Part by speaking to the Alchemist.

Back in Season 1, Malignant Hearts were items dropped by infected enemies to be used in equipment with Infested Sockets. These granted new abilities and unique effects, and there were a total of 32 Malignant Hearts in Season 1.

There were four types of Malignant Enemies: Vicious, Brutal, Devious, and Wrathful monsters. They are tougher than the average enemy and have different abilities. They are identifiable by their colors and drop a specific type of Malignant Heart.

Crafting and Salvaging Hearts in Season 1

In season 1, you can visit Cormond's wagon to salvage unwanted Malignant Hearts for Ichor. Ichor is a resource used in crafting Malignant Hearts. Salvaging a Vicious, Brutal, or Devious Heart will yield 5-15 Ichor, while salvaging a Wrathful Heart will yield 2-5 Ichor, as with all other Heart types.

To craft a random Heart of a specific type, you would need 35 Ichor of the other Heart types. For example, if you want to craft a random Brutal heart, you would need 35 Vicious Ichor and 35 Devious Ichor. This means that on average, you will need to salvage ~7 Hearts to craft 1 new Heart.
