Malignant Heart is a Lair Boss Key you can get from Whisper Caches and Helltide chests in Diablo 4 (D4). Read on to learn more about how to get Malignant Hearts, how to use it, and its general overview here!

How to Get Malignant Hearts
Obtained from Whispers Caches
The best way to get Malignant Hearts by completing the Whispers of the Dead quest and opening the cache reward from the Tree of Whispers.
How to Complete the Tree of Whispers
Open Helltide Mystery Chests
An alternative way to get Living Steel is from any Tortured Gifts found in the Helltides, particularly from the Tortured Gift of Mysteries.
Participate in Legion Events
Legion Events are a must-do for players as they spawn a high density of mobs and elites which makes them an ideal place to farm Malignant Hearts. Be sure to be on the lookout when these events show up on your map as they can also be a great place to farm items.

Undercity of Kurast with Tribute of Titans
The best way to get the Malignant Hearts by running the Undercity of Kurast with the Tribute of Titans for an increased drop rate of Lair Boss Keys. Keep in mind that you can get higher drop rates in Torment I or above so make sure your build is optimized enough to take on higher difficulties.
Astaroth Drops From Escalating Nightmares
Malignant Hearts can be obtained by killing Astaroth in Escalating Nightmares. Do note that although this rewards large quantities of Malignant Hearts, it's drop rate is based on the randomized affix, making this the least efficient farming activity.

How to Use Malignant Hearts
Used to Open Varshan's Hoard
In order to use your Malignant Hearts, you first have to defeat Echo of Varshan in the Malignant Burrow at Hawezar. Doing so will spawn his hoard chest inside the boss area where you can use 12 Malignant Hearts to open it.
Take note that Lair Bosses are now free to summon but their rewards now drop from their respective reward chests called hoards.

The Interactive Map both shows the location of key points on the map, and doubles as a collectible checker.
- Click an icon on the right side to show all the pins of a specific type, or click the magnifying glass button to search for a specific pin by name.
- Click a pin on the map to see its name and description, and a relevant picture or video if available.
- From here, click Mark as Acquired to indicate that you've gotten a collectible already. This will change its icon on the map to show that it has been collected, and add 1 to that pin type's Counter.
- To undo this, click on the same pin and click Restore to unclaimed.
- To check how many pins you have marked so far, check the Counter in the bottom left, and click the icon that looks like a checklist to see counters for all pin types.
- If the collected pins start to get in the way, you can hide them all by clicking the green Show Pins button on the left side.

 | All Pins This button is shown along with the Icons on the right when there are over 6 types of pins. Click it for a full selection of all available pin types. |
 | Counter Tracks the number of pins you have of a certain type. The pin type shown is set to the page and currently cannot be changed; however, all pin type counters can be viewed by clicking the checklist icon on the right side. |
 | View All Counters This icon is found on the right side of the Counter. When a map has more than one pin type, click this icon to see Counters for all pin types. |
 | Zoom In / Zoom Out This icon is found on the top left side of the map. You can press these buttons, or use your scroll wheel while hovering, to Zoom In and Out of the interactive map. |

Seasonal Mechanic That Grants Abilities
Back in Season 1, Malignant Hearts were items dropped by infected enemies to be used in equipment with Infested Sockets. These granted new abilities and unique effects, and there were a total of 32 Malignant Hearts in Season 1.

There were four types of Malignant Enemies: Vicious, Brutal, Devious, and Wrathful monsters. They are tougher than the average enemy and have different abilities. They are identifiable by their colors and drop a specific type of Malignant Heart.
Season 1 Release Time and What We Know
Crafting and Salvaging Hearts in Season 1
In season 1, you can visit Cormond's wagon to salvage unwanted Malignant Hearts for Ichor. Ichor is a resource used in crafting Malignant Hearts. Salvaging a Vicious, Brutal, or Devious Heart will yield 5-15 Ichor, while salvaging a Wrathful Heart will yield 2-5 Ichor, as with all other Heart types.
To craft a random Heart of a specific type, you would need 35 Ichor of the other Heart types. For example, if you want to craft a random Brutal heart, you would need 35 Vicious Ichor and 35 Devious Ichor. This means that on average, you will need to salvage ~7 Hearts to craft 1 new Heart.
