Wrathful Heart is a type of Malignant Heart that you can socket on Rings and Necklaces with Infested Sockets in Diablo 4 (D4). Read on to learn the complete list of Wrathful Hearts, learn its effects, class restrictions and how to get them.

Malignant Heart Class and Effect
Creeping DeathCreeping Death Class: General
Effect: Your damage over time effects are increased by 30-40% for each different Crowd Control effect on the target. Unstoppable monsters and Staggered bosses instead take 110-130% increased damage from your damage over time effects.
Ignoring PainIgnoring Pain Class: Barbarian
Effect: Incoming damage has a 5-15% chance of being ignored and instead Healing you for 17-68.
OmnipowerOmnipower Class: Sorcerer
Effect: Core Skills that launch a projectile consume all of your Mana. For every 45-35 extra Mana consumed, you launch an additional projectile, and the damage is increased by 3.0-5.0%.
The BarberThe Barber Class: General
Effect: Critical Strikes and all subsequent damage within 2.0-4.0 seconds is absorbed by your target. Then, the absorbed damage erupts onto surrounding enemies. Stored damage is increased by 10% per second.
The Great FeastThe Great Feast Class: Necromancer
Effect: Each Minion drains 1.0-2.0 Essence per second but deals 50-75% increased damage. With no Minions, this bonus applies to you and drains 5 Essence per second.
The Malignant PactThe Malignant Pact Class: General
Effect: Cycle through a Malignant bonus every 20 kills: Vicious: Gain 20% Attack Speed.; Devious: Core and Basic Skills have a 15% chance to fully restore your Primary Resource.; Brutal: Every 21 seconds, gain a Barrier absorbing 85-102 damage.
The Unconstrained BeastThe Unconstrained Beast Class: Druid
Effect: When you are hit with a Stun, Freeze or Knock Down effect, there is a 40-60% chance to automatically activate Grizzly Rage for 3 seconds.
The Vile ApothecaryThe Vile Apothecary Class: Rogue
Effect: Your attacks have a 5-15% chance to apply all Imbuement effects at 40-50% of normal potency.

How to Farm Wrathful Hearts

Fight the Echo of Varshan Repeatedly

When you defeat Varshan, the final boss of the Season 1 Questline, you will get a free Wrathful Heart as well as an Invoker of Varshan. You can use this item in a secret room in Malignant Tunnels in World Tier 1 or 2 to battle the Echo of Varshan (see: How to Use Invoker of Varshan).

Upon defeating the Echo of Varshan, you will receive multiple Malignant Hearts which have a chance of being a Wrathful Heart.

You can craft Invoker of Varshan at Cormond's Wagon after getting the first one. Use these to fight the Echo of Varshan repeatedly and farm Wrathful Hearts for your build.

Use Foul and Tortured Invoker of Varshan for Higher World Tiers

Fighting the first Echo of Varshan will also reward you with a Foul Invoker of Varshan which can be used in World Tier 3 to fight a stronger Echo of Varshan. Upon defeating that version, you'll get a Tortured Invoker of Varshan for use in World Tier 4 to fight an even stronger version. Each of these will yield higher level Malignant Hearts matching their World Tier, and can also be crafted at Cormond's Wagon after obtaining the first of each. Use these more powerful Invokers to continue to farm Wrathful Hearts as you approach endgame.

How to Use Invoker of Varshan

Join a Party to Share Wrathful Invokers

When using a Wrathful Invoker in a Malignant Tunnel online with other players, only one Wrathful Invoker is needed to make a Wrathful Heart spawn for all players.

If you're doing this with a party of 4, and each player takes turns using an Invoker, you'll each be able to get 4 Wrathful Hearts for each Wrathful Invoker, managing your supplies much more effectively. Wrathful Invokers are very rare, so take the time to share the effects with other players and they'll get you back when they have theirs.

Craft an Uncertain Heart

Currently, it doesn't seem possible to craft a Wrathful Heart directly. This might be unlocked in a later stage of gameplay; if so, we will add it when it is confirmed.

The only way to craft a Wrathful Heart is by crafting an Uncertain Heart, which has a 10% chance of resulting in a Wrathful Heart. This was confirmed in the Wrathful Invoker Hotfix on August 9, 2023.

Wrathful Malignant Monsters

Diablo 4 - Malignant Hearts Season 1
It's also possible to obtain Wrathful Hearts by defeating Wrathful Malignant Monters which can randomly spawn in dungeons. However, this method is random so it's not an efficient farming method for Wrathful Hearts specifically.
