The ending of Diablo 4 (D4) provides more questions than answers and leaves plenty of open ends. Check out our explanation of the ending and learn what happens to each main character and what the events could mean for the game's future.

As Neyrelle and the Wanderer (player character) reach the Cathedral of Hatred, they find Mephisto's essence. While their original plan was to use the Soulstone to trap Lilith, Neyrelle proposes that they seal Mephisto in the Soulstone instead and leave Lilith trapped in Hell.
While this is a risky proposal, given that Lilith could steal the Soulstone and take Mephisto's power for herself, the Wanderer puts her trust in Neyrelle, making her seal Mephisto's essence inside the Soulstone. Slowly being trapped in the stone, Mephisto remarks that they have "sealed [their] fate."
With Mephisto sealed in the Soulstone, the Wanderer decides to stay behind to stop Lilith, letting Neyrelle escape with the help of Mephisto's portal. As Neyrelle leaves, Lilith makes her entrance, attacking the Wanderer to signal the start of the climactic battle.
Despite Lilith's potent demonic powers, the Wanderer succeeds in taking her down. As she kneels in defeat, Lilith mentions that the cost of her defeat will be "more than [they] can pay" while visions of demons flash before the screen.
We speculate that these visions show one or two of the Prime Evils, namely Mephisto and Diablo, which may hint at their return in Diablo 4's future. In addition, while Lilith turned to stone and crumbled to dust in her defeat, the franchise's trope of villains coming back after death means that it's also possible for her to return in some capacity.
After defeating Lilith, the Wanderer returns to Lorath but finds that Neyrelle and the Soulstone are missing. They return to the Forsaken Chapel to look for her but find nobody there, surmising that she might be in the Horadric Vault instead.
However, as they leave, Iosef, one of your allies from Act 1, and a small band from the Knights Penitent confront Lorath and the Wanderer before attacking them. After taking them out, they find that Reverend Mother Prava decreed the Horadrim to be enemies, wanting Lorath and the Wanderer dead and the soulstone in their possession.
We speculate that Prava's decree and these events in the ending are the canonical explanations for the Cathedral of Light Capstone Dungeon only appearing after completing the campaign, as the dungeon is located in one of the faction's cathedrals and heavily features Knights Penitent enemies.

After being attacked by the Knights Penitent, Lorath and the Wanderer temporarily part ways, as the former needs to bury Donan in Scosglen while the latter needs to look for Neyrelle in the Horadric Vault. Upon reaching the Vault, the Wanderer finds a letter from Neyrelle penned in Horadric Script.
When the two reunite and read the letter at the Firebreak Manor, they find that Neyrelle has left on her own, meaning Lorath Nahr is the last Horadrim left. Neyrelle also mentions in the letter that her journey involves "facing him and his brothers," alluding to Mephisto and the two other Prime Evils, Diablo and Baal.
From this ending, we assume Neyrelle went on a solo journey to find answers to stop the possible return of three Prime Evils now that she has the soulstone of Mephisto. However, with Mephisto's wolf form stalking and following Neyrelle in the cutscenes, it's also possible that Mephisto could be manipulating her even while sealed inside the Soulstone.
In addition, the image of a wanderer carrying the Soulstone of a Prime Evil calls back to the events of Diablo 1 and Diablo 2, where Aidan, also known as the Dark Wanderer, attempted to seal away Diablo's soulstone by plunging it into his forehead, only to become the Lord of Terror's vessel in the future. Whether this is just a parallelism by Blizzard or a possible hint of things to come is not yet known.
