To open the door inside Malignant Tunnels, you first need to finish the Season 1 Questline in order to get an Invoker of Varshan.

Diablo 4 - Can Use an Invoker of Varshan to Summon Echo of Varshan

Behind the door is a Monstrous Outgrowth where you can use the Invoker of Varshan to summon Echo of Varshan. Echo of Varshan is similar to the Varshan encounter in the Season 1 Questline, but much deadlier.

Defeating Echo of Varshan on World Tier I or II will give you a recipe to craft a Foul Invoker of Varshan, while on World Tier III you will get a recipe for a Tormented Invoker of Varshan. Crafting these Invokers allow you to farm the encounter for Malignant Hearts, but can only be used in certain World Tiers.
