Speak to Phelan
Talk to Phelan in The Downs to start the quest.

Diablo 4 - Place the First Wardstone
Place the First Wardstone
Head south from Phelan and place the first wardstone down.

Diablo 4 - Place the Second Wardstone
Place the Second Wardstone
From the first wardstone, head northeast to place the second wardstone. Defeat the enemies in the area so that they do not disturb you while you place down the wardstones.

Diablo 4 - Place the Third Wardstone
Place the Third Wardstone
Head south and place the final wardstone.

Diablo 4 - Return to Phelan
Return to Phelan
After placing all three wardstones, return to Phelan to progress the quest.

Diablo 4 - Unhallowed Pits
Travel to the Unhallowed Pits
Travel to the Unhallowed Pits to find Blackthorn's lair.

Diablo 4 - Slay Blood Clan
Slay Blood Clan Khazra to draw out Blackhorn
Slay the Lycan enemies on the way to Blackhorn's lair to draw him out. Defeat the enemies within the highlighted area to contribute towards filling the quest progress bar.

Slay Blackhorn and acquire his head
Slay Blackthorn and the other enemies in the area. Once Blackthorn is defeated, pick up the Head of Blackthorn from the corpse.

Return to Phelan
Return to Phelan in The Downs to finish the quest.
