Sister Octavia is an NPC in Kyovashad who can be found in the alley between the Healer and the Curiosities Vendor. Speak to her to begin this side quest.

Objective

Diablo IV - Malady of the Soul Sister Octavia
Speak to Sister Octavia, who can be found conversing with Petr and will ask you for assistance regarding an exorcism.

Follow Sister Octavia into the Frigid Cellar within the house she was just standing in front of.

Once inside, speak to Sister Octavia to receive the Holy Chalice quest item, and place it near the boy on the bed.

Defeat the wave of demons that spawn during the ritual.

Slay Xul'goth, who will spawn after defeating the initial wave of demons.
6 Diablo IV - Completing Malady of the Soul
Exit the Frigid Cellar and speak to Sister Octavia to complete this side quest.
