The Butcher's Cleaver is a Unique item drop that can only be obtained by defeating the Butcher while playing at World Tiers 3 or 4. Defeat him when he appears in your dungeon to get a chance to obtain this Unique weapon. This item is not guaranteed to drop if you do beat him though, as the Butcher can also drop other Unique items and legendaries.
