Dropped by the Butcher at Nightmare and Torment Difficulties

The Butcher's Cleaver is a Unique item drop that can only be obtained by defeating the Butcher while playing at World Tiers 3 or 4. Defeat him when he appears in your dungeon to get a chance to obtain this Unique weapon. This item is not guaranteed to drop if you do beat him though, as the Butcher can also drop other Unique items and legendaries.

Need to update the post for this cleaver, it not only drops from the Butcher, but i got it from the world boss just now as well.
