Nostrava is one of three Strongholds found in Fractured Peaks. It is located to the west of Kyovashad and must be liberated before the Sight to Madness sidequest can become available.

Sight to Madness Walkthrough

Objective

Diablo IV - Sight to Madness World View
Speak to Vilek, who can be found in the northern side of Kyovashad. You'll find Vilek bound by a pillory amongst other pillories and an assortment of cages.
2 Diablo IV - Cultist
Teleport or walk to Nostrava to reach the Cultist's Refuge entrance. This dungeon can be found within the chapel, near where you fought the three succubi to liberate Nostrava.

Find and slay the Elite monster Theya to obtain the Necklace of Eyes quest item.

Head back to Vilek to complete this quest, return his eyes, and to see what happens after he tries to put them back into their sockets.

This is by far the worst quest so far. No way of starting it, and you die as soon as you go into this chappel. I'm considering a rage quit.
