The Helm of Urtkot unlocks after completing Percival Rackham's Trial. You must also reach level 12 and learn Depulso from Professor Sharp's Assignment 1 first before you can start the quest.

Description | After I managed to track down the goblin named Lodgok in the Hog's Head Inn, he told me that he wants to retrieve a sacred goblin relic stolen years ago by a witch. Rumour had it that the relic was in the witch's tomb. Unfortunately for Lodgok, the tomb is only accessible by wizardkind. |
Tracking the quest leads you first to Hogsmeade. Fast travel to the South Hogsmeade Floo Flame and talk to Sirona inside the Three Broomsticks, who will tell you where to find Lodgok.
Lodgok can be found at the Hog's Head Inn, on the southwest corner of Hogsmeade. Talk to him inside to learn about the relic that he wishes to retrieve to gain Ranrok's trust.
Go outside Hogsmeade through the bridge near The Magic Neep and meet Lodgok at its outskirts.
Walk with Lodgok until you reach the tomb west of Hogsmeade. You might also encounter a Dark Mongrel near its entrance so be prepared.
Upon entering, go straight ahead to find a door with two moths at the center. This is a moth door that can only be unlocked after three moths are attached on it.
The rooms inside the tomb are locked by moth doors that you can open by gathering three moths with Lumos. Cast Lumos near a perched moth to make it follow you. You can then release them with the Basic Cast or recasting Lumos near a locked moth door, or a spinner so you can interact with it using Depulso.
Have Lumos on your Spell Set throughout the quest to quickly solve the moth door puzzles, and also since the tomb doesn't have enough lighting.
Now, from the first moth door, turn right and check the end of the path to find a lone moth. Use Lumos to attract the moth and take it back to the door. Enter the next area and before you proceed futher, check the small room to the left to find a Chest.
Follow the path until you reach a room with another moth door. Find two of the moths on either side of the room and take them to the door with Lumos.
The third moth is hidden behind the broken door. Break down the door with Depulso to find a small Chest and the moth. Take the moth back to the moth door to open it.
In the next room, you'll fight a horde of Inferi. They are immune to damage until they are burned. Cast Incendio or Confringo on them first to make them vulnerable before using your other spells. You can also throw at them the flaming pots in the room to inflict the same effect.
Continue ahead and you'll end up in a room with a moth spinner in the middle. Take one of the moths on either side of the room, attach it to the spinner, then hit it with Depulso to raise a platform nearby which you can use to reach the opening above it. Climb to the opening and bring the third moth to the door. Return to the spinner and bring the moth from earlier to the moth door to open it.
Immediately upon entering the next room, climb the ledge on your right and follow the tunnel that will lead you to another room with a large Chest on the other side. Using Revelio in this room highlights the bone piles that you will need to create a bone bridge to the chest.
Pull the bone piles with Accio and place them on the rectangular (between the poles with skulls) tile to construct a part of the bridge. You will need to place two bone piles placed to complete the bridge and reach the Chest.
Return to the previous room and proceed to the next. Use Basic Cast on the hanging weight to knock a hole in the floor that leads to the final puzzle room.
Before you drop down, check the door on the right and blast it open with Depulso. Climb the box to find a large Chest then go back down. Drag the box with Accio and place it below the other small room to reach the Collection Chest containing Natural Terrain.
Go back to the main room and fall through the hole on the floor. The tunnel behind you leads you back up to where you fell from, so go straight ahead and fight the Inferi in the final puzzle room.
Let's loot the area first before solving the final moth door. Check the right side of the room for a small Chest. Gather the moth behind you with Lumos and place it on the spinner at the center. Stand on the nearby platform and hit the spinner with Depulso a few times to raise the platform.
Turn east past the other moth to find another small Chest. To your left is a tunnel that leads to a small room with the last two Chests.
Go back up and hit the spinner again with Depulso until you can use the platform to cross to the other side. Before the platform descends, lure the moth next to you with Lumos and take it to the moth door on the other side.
Take a few steps back to find the second moth just before the platform and take it to the moth door. Go back down for the third moth.
The last moth is in an alcove in the southeast corner, and you need to first place it on a perch on the east side of the room.
Go back up, use Depulso on the spinner again if necessary, grab the moth from the ledge, and place it on the door. Interact with the body on the sarcophagus to complete the objective. Open the Chest on your right before leaving through the hidden path behind the sarcophagus.
Speak to Lodgok outside and tell him what you found inside the tomb.
Fly to the quest marker to find Ashwinders fighting a Troll. Here, you can ignore the thieves and simply sneak around the camp while under the effect of Disillusionment then grab the helmet from the chest inside the tent on the north.
Enemy reinforcements will spawn as soon as you grab the helmet, but you can hop on your Broom to make a quick getaway.
Fly back to Lodgok and return the helmet to finish the quest.
