The choice affects whether Sebastian appears in the ending of the main story and in the final quest to conclude his questline, In the Shadow of Friendship. If you turn him in, Ominis will take his role in those aforementioned concluding scenes.

Sebastian can still teach you the curses

If you don't learn the Unforgivable Curses during their respective quests, you can still learn them from Sebastian by talking to him in the Undercroft. Consequently, if you turn Sebastian in, then he will no longer show up in the Undercroft to teach you the curses.

Given that Sebastian is remorseful and will have to live with both the guilt and being estranged from his twin sister, you can choose to not turn him in so that he's present at the conclusion of both the main story and of his own questline.
