The Poacher's House Call is unlocked as part of Poppy Sweeting's Relationship questline. In order to unlock it you'll first need to have completed all of the previous quests as listed above.

Poacher
Talk to Poppy.
Poppy is south of the Belltower Wing just outside of Hogwarts Castle. Head south from the Beasts Classroom Floo Flame fast travel point.

How to Complete the Quest

After talking to Poppy, the quest will oddly just end without a Quest Completed screen. Don't worry, you completed the quest and the storyline will continue in Surprise Meeting.

Centaur Choice Doesn't Matter

Choice Result
Let's talk to the centaurs. Poppy will decide we need to go to the centaurs.
Centaurs may be too hostile to help. Poppy will decide we need to go to the centaurs.

There's not difference in outcome whether you choose to go the centaurs or not. In fact, Poppy replies with the exact same dialogue to both options: "We'll need to be careful in how we approach them. Tensions with them are high."
