Start the M'ap Tann Pelen mission and find Placide in Rolland’s Butcher Shop. Before taking the Butcher's Cleaver he left on the table, speak to him and wait for him to finish butchering the chicken.
Written on the 1.6 Patch Notes, the standard Butcher's Cleaver will be replaced by an iconic version. If a player owned any Butcher's Cleavers that weren't iconic, it will be replaced with a Knife along with the recipe of matching rarity.
( You can't get the cleaver after the quest.)

I found mine as the cleaver Placide uses to kill the chicken. After he chops his head off and tells you to follow him, he left the cleaver on the table and it became lootable.
