How to Get the Scalpel

Get in the Big in Japan Quest

You can get the Scalpel by completing the quest Big in Japan. To unlock the Big in Japan side quest, talk to Dennis at Afterlife in Act 2. He should be located right in front of where Rogue is sitting. He will ask you if you are interested in some work.

Get the Legendary Version via Crafting

After getting the Scalpel from the Big in Japan side quest you will be able to craft the Epic and Legendary versions. Crafting will require the Grease Monkey Perk and the Edgerunner Artisan Perk as these allow you to craft Epic and Legendary weapons, respectively.

This is obtained during an act 2 side quest in Kabuki. I cannot remember the name of the side quest however it involves retrieving a man from a refrigerator near the docks I think. When the quest is done this is your board
