The Sacrificial Heart is a quest item that is dropped by one of the enemies found in Aggorat. This area is set in Act 3's past and is accessible after defeating Ketzuli, High Priest of the Sun and passing through the Gateway in Act 3.

Reset Instance to Spawn New Enemies

If the Sacrificial Heart did not drop after clearing the area, you can reset the instance by using a waypoint, then hit control + left click on Aggorat on the map to create a new instance. This will reset the area and spawn a new map with fresh enemies.

How to Use Sacrificial Heart

Sacrifice at the Ritual Site in Aggorat

Step	Objective
1	Pick up the Sacrificial Heart that drops randomly within Utzaal.
2	Proceed to the Sacrificial Dais in Aggorat. Note that maps are randomly generated and this is only a sample layout meant as a general guide.
3	Pick up the Sacrificial Dagger on the altar, place the Sacrificial Heart, and stab the Sacrificial Heart.

The Sacrificial Heart is used by interacting with a ritual altar in Aggorat. This altar is always found in the Trail of Atziri past the chanting Vaal in front of the Atziri statue. The altar seems to spawn most often in the northeast section of this area.

Once you reach the altar, defeat the surrounding enemies, pick up the Sacrificial Dagger and select the Blood Sacrifice option on the altar. You will then receive two Weapon Set Passive Skill Points.

Can be Missed After Moving to Act 1 Cruel

If you missed the Sacrificial Heart or have progressed to Act 1 Cruel without sacrificing it, getting back to Aggorat or Utzaal can be confusing. These areas are no longer accessible normally through the waypoint map once you move forward. Fortunately, there’s a workaround that involves going through the gateway in the Ziggurat Encampment.

How to Get Back to Aggorat or Utzaal

1. Use the Ziggurat Encampment Gateway
2. Waypoint-Travel to Aggorat or Utzaal

Go Through the Ziggurat Encampment (Present) Gateway

Once you reach Cruel difficulty, it's not easy to get back to the areas set in Act 3's past. To return to Aggorat or Utzaal and complete the Sacrificial Heart quest, head to Ziggurat Encampment (Present) and click on the Gateway. Although it should send you to the past, it will take you to The Riverbank in Act 1 Cruel once you've completed Act 3. At first glance, this might not seem helpful—but this somehow changes your Act 3 map to the Past version of the area.

Find a Waypoint and Teleport to Aggorat or Utzaal

Once you're in The Riverbank, get to a Waypoint—we recommend using the one in Clearfell Encampment. After opening the Waypoint-Travel map, your Act 3 map will now be set in the past and include access to Utzaal, Aggorat, and the Black Chambers.

Use the waypoint to teleport to Utzaal if you need to collect the Sacrificial Heart, or head to Aggorat to find the altar and complete the quest that grants two additional Skill Points.
