Same, all cleared, no Gharl. On Tier 3. Update: I abandoned the quest, switched to Tier 4 and it worked.

Anyone else have THIS issue? I've killed everything in the cave up until the red blood wall that blocks off the rest of the cavern. The wall will not go away. I've destroyed absolutely anything that I can, and have the chieftain with me but nothing happens. I can't complete the quest.
