At the coast of Puffer Breach, there is a overhead cave near the rocks. Attach a Rocket Zonai Device on a rock and use it to go up the cave.
Continue heading deeper into the cave and you will find a chest that contains the Phantom Helmet.
You can buy the Phantom Helmet for 400 Poes from a Bargainer Statue in the Depths after getting the armor first in-game.
