How to Get the Labyrinth IV

Obtain from Doctor's Secret Room in Kaymanov Ending

Labyrinth IV Overview

In order to get the Labyrinth IV, you need to go through the Kaymanov ending where you choose to not shoot the Doctor and instead lower your gun in the main mission, The Last Wish. The gun is located inside the secret room accessed from the Doctor's basement.

Do note that the Labyrinth IV cannot be used in a regular playthrough as it is obtained after you pass the point of no return in Down Below. You can still use the Labyrinth IV during the final mission of Kaymanov's route.
