The Religious Icon Choice will happen after getting the icon from the Root Cellar during the The Poppy Field mission. Giving it to Pomor or Mityay will affect the rewards you can get after the mission.
Choosing who to give the Religious Icon to will depend on what you may want or need for your playhthrough.
If you want a decent weapon, trade the Religious Icon to Pomor to get the Unknown Stalker's AR416. However, you will have to repair it first, as it has low durability when you receive it. If you only want to earn Coupons, give the Religious Icon to Mityay instead.
