You can only access the Church of Dragon Communion in Limgrave after completing the Coastal Cave dungeon and defeating its boss. Once you've done that, travel through the tunnel in the back of Coastal Cave's boss room and you'll eventually end up on a small island with the Church of Dragon Communion.
