You will need the Flask of Wonderous Physick from the base game first to start mixing and using Crystal Tears. You can find the Flask of Wondrous Physick at the Third Church of Marika. Look for the large bowl on the floor near the Site of Grace and examine it to obtain the item.

Mix Crystal Tears at a Site of Grace

Elden Ring - Rest at a Site of Grace

At a Site of Grace, look for the Mix Wonderous Physick option to access the mixing menu.

Choose two crystal tears to create a flask with custom effects. For example, if you have a Crimson Crystal Tear and Greenspill Crystal Tear, you can mix them together to create a Flask that restores HP and temporarily boosts stamina.
