From the Gravesite Plain Site of Grace, head towards the ruins northeast to find the Scorched Ruins. Gravesite Plain is the area and Site of Grace you start the DLC in.
The Scorched Ruins Site of Grace is located in the northern side of the ruins. Pass through the ruins under the arch and you'll find the Site of Grace nearby on the right side of the road.

You can find the Gravesite Plain Map Fragment just before reaching the Scorched Ruins from the south side. Take this to unlock the map for the surrounding area.
All DLC Map Fragment Locations
On the right side of the ruins facing north, you can find a corpse on the first platform which gives you a Hefty Cracked Pot.
Continue up the stairs on the right side and you'll find another corpse containing the Greater Potentate's Cookbook (1) on a set of stairs that leads to a closed door.
On the left side of the ruins facing north, you can find a path that leads to a broken building filled with pots. Break the pots to reveal a corpse containing Shadow Realm Rune (2). Beware of an aggressive dog that will attack you.
Head up the stairs in the broken building and exit through the balcony. Jump on the roof to the right on the far side to find a corpse containing Furlcalling Finger Remedy. Be careful of enemies that throw stones at you from the building across.
Jump on the roof to reach the building across where the stone-throwing mobs are. Continue up the stairs and through the rooftop to reach a door with a chest inside. The chest contains the Blade of Mercy Talisman.

From the room with the chest, jump towards the balcony on the left to reach a corpse containing Redflesh Mushroom.
Again, from the room with the chest, drop through a missing roof section to reach a building with a corpse containing Black Pyrefly.
A Furnace Golem can be found and seen north of the Scorched Ruins past the Site of Grace. Be careful getting too close and getting aggro from the golem, it shoots dangerous ranged attacks.
