Coming west from Limgrave, follow Caelid Highway east. As the main road turns south, Caelem Ruins appears just after Caelem Ruins Site of Grace.

When Pumpkin Head swings down, dodge diagonally forward, so you end up behind it. Then hit it once or twice, and continue circling around. Repeat this process until Pumpkin Head dies.

This time, you'll face two Mad Pumpkin heads, but the same basic strategy applies. One Mad Pumpkin head wields a Flail, while the other wields a hammer. Defeating the duo gets you a Visage Shield and 6300 Runes.
