Obtained Through the Burning Altar in Ruined Forge of Starfall Past

The Ancient Meteoric Ore Greatsword can be obtained from a burning altar near the end of the Ruined Forge of Starfall Past. From the Site of Grace, proceed to the first area and use the lever to unlock the blocked path. Traverse through two corridors until you see a ladder to your right.
Climb up the ladder and press on until you reach a room filled with molten lava. Run past the room and drop down the elevated walkway to reach another blocked area, which can also be unlocked by a lever.
Once the blocked path has opened, enter and use another lever found on the right to lift the previous contraption. Jump on top of the rising wall to reach the room with the greatsword.

From the Castle Front Site of Grace, follow the path southeast to reach the Pillar Path Waypoint Site of Grace. Continue along the path southwest until you reach Dragon's Pit.

The Smithscript Greathammer is found in Taylew's Ruined Forge. From the Site of Grace, continue inside and turn left. Climb down the ladder and drop down the gap in the next room.
Turn right, jump onto the column, and hop on the ledge opposite of where you came from. The Smithscript Greathammer is just down the hall to the left.

How to Access DLC Strength Weapons
Requires Shadow of Erdtree DLC
You can access all the DLC unique weapons in the Land of Shadow by interacting with Miquella's Egg at the Cocoon of the Empyrean Site of Grace in Mohgwyn Palace after defeating both Starscourge Radahn and Mohg, Lord of Blood.
Note that you would have to purchase and install the Shadow of Erdtree DLC to be able to reach the Land of Shadows.
