Hornsent Grandam can be found inside the Locked Storeroom of Belurat, Tower Settlement. Before you can speak to her, you will need to find the Storeroom Key that unlocks the room she's in.
From the Small Private Altar site of grace, head north and go through the narrow alleyways until you find a canal. Upon reaching the canal, keep facing west and follow the wall until you see a shadowed doorway. Head inside and climb the ladder in the room. At the top of the ladder will be a seated corpse that has the Storeroom Key.
With the Storeroom Key secure, backtrack to the Small Private Altar site of grace. This time, instead of heading north after exiting west, go southwest onto the walkway with 2 Shadowed Casters. Defeat them then open the door at the end of the walkway to unlock the Storeroom.
Inside, speak to Hornsent Grandam. She will rebuff you the first time you speak to them since they'll mistake you for one of Messmer's soldiers.
Hornsent Grandam will only speak to you if you're wearing the Divine Beast Head helmet. To get this legendary armor piece, you will need to defeat the Divine Beast Dancing Lion at the top of Belurat, Tower Settlement.
On death, this remembrance boss will drop the Divine Beast Head on the ground. Make sure to pick it up before leaving the area.
How to Beat Divine Beast Dancing Lion
With the helmet secure, return to the Belurat Storeroom and wear the Divine Beast Head. Speak to Hornsent Grandam while wearing the helmet and she will reward you with the Watchful Spirit incantation.
Next, reload the area by resting at a site of grace. Once reloaded, head back to Hornsent Grandam (while wearing the Divine Beast Helm) and speak to her again. This time, she will reward you with some Scorpion Stew.
You can take the Scorpion Stew back to Hornsent at the Three-Paths Cross site of grace. In exchange for the nostalgia-laden soup, Hornsent will give you 3 Furnace Visages.
Finally, return to Hornsent Grandam after you defeat Messmer, the Impaler. Wear the Divine Beast Head once more to initiate dialogue and she will reward you with a unique item called Gourmet Scorpion Soup.
The Gourmet Scorpion Soup is a consumable item that temporarily enhances physical damage negation and gradually restores your HP. Currently, these are the only two that can be obtained in-game.
To get your second drop of Gourmet Scorpion Soup, reload the area again before returning to Hornsent Grandam. This time, she will be asleep. Loot the soup in front of her to end her questline.
You will need to be wearing the Divine Beast Head armor piece that you can obtain by defeating the Divine Beast Dancing Lion for Hornsent Grandam to talk to you.
The Divine Beast Dancing Lion can be found in the Belurat, Tower Settlement Legacy Dungeon, located west of the Gravesite Plain.

Speaking to Hornsent Grandam with the Divine Beast Head equipped will net you various rewards after defeating the Divine Beast Dancing Lion and Messmer the Impaler.
The Watchful Spirit Incantation and Scorpion Soups can be obtained after you defeat the Dancing Lion. While the Gourmet Scorpion Soup can only be obtained after defeating Messmer.
