The Chicken Hat reduces damage taken from enemies. In the table above, it shows how much damage is taken when attacked by a Ganado on Professional difficulty. To put it clearer, normally you can only take 2 hits from a Ganado without the Chicken Hat. However with it equipped, damage is reduced enough that you can withstand 3 blows.
This damage reduction perk provided by the Chicken Hat makes it an essential accessory for any player who wishes to attempt an S+ playthrough on Professional difficulty.
Professional S+ Guide: How to Speedrun Professional Mode

The Chicken Hat is a special unlockable accessory that you gain as a reward when you complete the game on Hardcore or Professional with a rank of S+ meaning a new game playthrough of under 5 hours and 30 minutes.
You can equip this accessory from either the Pause Menu or Main Menu. Go to Extras in either menu then select the character this accessory belongs to in order to make it part of your in-game outfit!
The perk that comes with this Accessory only activates while you have it equipped. If you remove this Accessory, the perk's exclusive stat bonus will deactivate until equipped again.
