Complete in-game achievements to rank up and unlock Trainer Card stars and colors. Read on to learn how to get trainer card stars and badges, as well as what is the trainer card in Pokemon Brilliant Diamond and Shining Pearl (BDSP).

Your trainer card changes color schemes 6 times, corresponding to each star you unlock by attaining certain achievements in the game. Completing the achievement gets you 1 star and can be done in any order.

After obtaining all 5 Stars on your Trainer Card, the Nurse will recognize this on your next visit! It's a nice little empowering reward that you can enjoy being recognized for your efforts!
After the initial recognition, succeeding visits to the Pokemon Center will have a more familiar and casual dialogue between the player and the nurse. What a nice touch!
To get badges, simply defeat the Gym Leader assigned to each badge. Gym Leaders are arranged on the Badge Case in the order you challenge them. Through the trainer card, you can view your Badge Case, which displays portraits of all the Gym Leaders and their respective badges below that. You can use this to view the badges you've collected so far.
Additional effects of the Gym Badges include producing a sound when tapped, akin to musical notes. Badges can also be wiped to the point where they sparkle.The badges become faded if left neglected for a long time.

A Trainer Card showcases your progress in collecting Gym Badges and trainer card stars. On it, you can view your Player ID, how many Pokemon are registered in your Pokedex, how much money you currently have, when you started your save file, when you first entered the Hall of Fame, and how long you've been playing.
As of Ver 1.2.0, Trainers can now view your Trainer Card in the Union Room. You and other Trainers can view each other's Battle Tower Records as well.
This was likely just coincidence. As others have pointed out, having 100 different Underground statues gets the other star. I have said star but don't yet have all the apps.

Just found out today that you can get this scene by having only 4 stars (Gold) trainer card, and she'll be just as impressed. Sad to know that when I get that Black Card, it won't come with anything new.
