Distilled Paranoia is dropped by enemies found in the Delirium endgame activity. Deliriums spawn waves of enemies where you'll need to defeat as much of them as possible and can be found in the Atlas of Worlds.

Distilled Paranoia can also be obtained by visiting the Currency Exchange NPC in every Act. Currency Exchange becomes available in Cruel Difficulty which is unlocked after completing Act 3 on Normal difficulty.

Distilled Paranoia is used by right-clicking it to open the Instilling menu and then applying a Notable Passive to an Amulet.
