A Medusa can be found at the Caliginous Depths, located to the southwest of Bakbattahl in the Battahl Region.
You are close to its lair when you start seeing petrified statues on the way there. Make sure to have Malleating Elixir to cure petrification. This curative can be made by combining Cinnamon Bark and Goldthistle.

How to Beat the Medusa |

If you have the shield, Daughter of the Evening, you can reflect Medusa's petrifying attacks, instantly defeating it. Doing so earns you the Eye for an Eye Trophy.
Trophy Guide and List of Achievements
We recommend grabbing and holding on to Medusa's head to try and decapitate it. Climb up from the back to slash its head, or try knocking it down to lower its head.
When equipped with slashing weapons, you can get Medusa's Head as an item to use against enemies. You will also get the trophies Getting a Head and Off with Its Head! when you defeat a Medusa this way.
Medusa is weak to Lightning, so we recommend having Mage Pawns that can buff your weapon with Lightning Boon.
While we recommend attacking its head, attacking Medusa from behind is a safe way of fighting it. Sticking behind Medusa allows you to face less bow and petrification attacks.
You can cut off Medusa's head by attacking its head with slashing weapons. We recommend knocking Medusa down to attack and cut off its head in a single blow.
If you decapitate Medusa when it has 3 or more HP bars left, you will receive Preserved Medusa Head. At 1 HP bar and below, you will obtain Medusa Head or Decayed Medusa Head instead.
Medusa's head can decay after the passage of time, or through multiple uses. Once decayed, Medusa's head will lose its power to petrify enemies. Preserve it by depositing it at an Inn, and use it sparingly against tough enemies.
