You need to cut off a Medusa's head to obtain its item form. This is best done using slashing weapons like the Warrior's greatsword.
The Medusa's head will be high up in battle stance, so try climbing from the back and repeatedly slashing its head. You can also try knocking it down to lower its head.
The enemy is weak to lightning, so it would be good to have Pawns who can buff your weapon with a Lightning Boon.
A Medusa can be fought at the Caliginous Depths which is located to the southwest of Bakbattahl in the Battahl Region.
You will be able to tell your are close to its lair when you start seeing petrified statues on the way there. Make sure to have Malleating Elixir to cure petrification. This curative can be made by combining Cinnamon Bark and Goldthistle.

A Decayed Medusa Head may be found at a chest inside the B1F Vernworth Castle Vault guarded by Azorka. The access to the basement is in the hallway near the Vernworth Castle Kitchens.
To access the vault, you will need the vault key you earn from Oskar upon completing the Tolled to Rest quest at the Ancient Battleground. This key is also used to obtain the battleground vault containing the Infernal Edge duospear.
Wait for the guard at the vault door to walk away and then enter using the vault key. Sneak in and open the chests inside to get the item. One of the chests will also contain 20,000 G.
The Medusa's head is an implement that can be equipped to your character like a Lantern. Attach it and aim it at an enemy to unleash a petrifying beam of light that slowly turns any enemy into stone.
This is effective on bosses such as the Cyclops, Griffin, and even the Drake. Completely petrifying them and then striking them will immediately defeat the enemy, making it an effective one-shot kill item.

Medusa's head, while preserved, will eventually decay after repeated usage and the passage of time, losing its power to petrify enemies. To preserve the item, deposit it in a storage at an Inn.
Make sure to use the item sparingly such as using it only for difficult boss fights or as a last resort.
