Akakios will walk up to you as you pass through the Mercantile Ward in Bakbattahl, and asks you to deliver a letter to Ernesto in the Volcanic Island Camp.

The Twixt a Rock and a Hard Place starts in the Mercantile Ward in Bakbattahl.
Go to the Volcanic Island Camp and find Ernesto. Deliver him the letter from Akakios, and he'll ask you to deliver his letter to Barbas at the Excavation Site.

The Excavation Site is located southeast of the Volcanic Island Map. Go through the site's gates to find Barbas and deliver him the letter from Ernesto, which completes the quest.

Due to the frequent battles occurring in the Volcanic Island Camp, there is a chance that Ernesto might've perished during one of them. If you can't find Ernesto in the marked quest area, chances are he's in the morgue in Bakbattahl.
